Your First Visit
Your initial dental evaluation with Dr. Hanneman or Dr. Leeman will consist of a complete oral examination and any necessary digital X-rays to determine the current health of your mouth. We will then offer you any treatment options that may be warranted.
Our Approach
If your situation is more complicated, the doctor may request that we take digital photos and study models to help with our diagnosis and plan of treatment. We may ask that you return for a separate consultation visit with the doctor to review findings and discuss your options.
If your needs are routine, a plan can be attained immediately.
If possible, please provide our dental care team with the following information before your first appointment:
- Your dental X-rays (within last five years)
- A completed new patient form with office and health information. A list of medications you are presently taking is helpful. These forms can be found and printed here.
- If you have medical or dental insurance, bring your current ID card. This will save time and allow us to help you process any claims.
- Please alert the office if you need to take a dental pre-medication of antibiotic or anti-anxiety medications.
Financing & Insurance
For treatments not covered by insurance, we accept cash, check, credit or debit card at the time of service. When payment of the entire amount is made the same day as service, a 5% discount will be issued. This includes the use of MasterCard or Visa.
CareCredit Financing

Dental Insurance
Dental insurance has been helping people with their dental needs for many years. If you have dental insurance, our staff will file your claims for you electronically. We will estimate any deductibles or co-payments that you are responsible for and ask for payment of those at the time of service.
Unfortunately, if you have many complicated dental needs, your insurance won’t cover everything. However, the good news is, it will still help!
Many insurance plans will have lists of dental providers. However, by no means does that mean that you can only receive care and ultimately insurance reimbursement for that care, from the dentists on “the list”. Most dental plans have provisions for “out of network” reimbursement that are many times similar to “in network”. You’ll find that dental insurance plans are usually quite different from medical insurance plans.
